COMMUNITY SOCIAL WORK

If there is a situation causing you concern, we are available for, advice, advocacy, assistance, referral, and support. The concern may be related to a health or social need, or it may be a family member or friend you are worried about.

We provide a service that is specialised in supporting those 65 and over, their family/ whānau and the wider community. Our Team often works as navigators to support you through the complex healthcare and social service systems, ensuring that older people have access to information and services available to them.

The team have a wide knowledge of the needs of older people and their carers, we work closely with other community-based organisations and health professionals.

What we do?

Our service is designed to meet the needs of those who find themselves in need due to the adverse circumstances that can arise with ageing.

This may be the individual, family/whanau or the wider community. This could include but it is not limited to:

  • Provide advice and assistance to access entitlements and supports
  • Welfare checks and assessment
  • Support to access social activities for companionship and to reduce social isolation
  • Support with End-of-Life planning or Enduring Power of Attorney
  • Transition to Rest Home support
  • Provide information on all aspects of ageing 
  • To support organisations to work with older people so they have an inclusive and positive experience with providers and agencies
  • Participate in community and awareness events.
